When patients first hear the term, many imagine dramatic cracking sounds and forceful movements. In reality, chiropractic adjustment is a precise, controlled, and remarkably gentle technique that has been practised and refined for over a century. First, joint manipulation directly restores movement to restricted spinal joints, reducing mechanical stress on the joint capsule, disc, and surrounding soft tissues. Second, spinal manipulation activates mechanoreceptors and proprioceptors within the joint, producing a rapid neurological effect that inhibits pain signalling, a mechanism known as gate control. This is why patients often experience immediate pain relief following an adjustment.

Third, spinal manipulation has been shown to reduce muscle guarding and spasm in the muscles surrounding the treated joint. This is particularly relevant in patients with acute back pain, where protective muscle spasm can perpetuate pain and restrict movement long after the initial injury has healed. Fourth, adjustments to the cervical spine have been shown to influence the nervous system more broadly, which explains the effectiveness of chiropractic care for conditions such as cervicogenic headaches and tension-type headaches.

The most commonly reported side effects are mild soreness in the treated area for 24 to 48 hours after the adjustment, which is similar to the muscle soreness experienced after exercise.
